GERIATRIC CARE MANAGEMENT SERVICE​S 

Aging in Place initial evaluation including a home safety assessment with recommendations, and review of options to maximize safety, comfort and independence in the home.

If there is a need for expanded services in the home, levels of supportive as​assistance including companion and home health care services would be identified. Potential eligibility and need for in-home skilled nursing / therapy could also be reviewed.

For persons looking to downsize or move to a supportive congregate residence, assessment of eligibility factors for independent, assisted living, expanded assisted living services or nursing home admission criteria can be part of the initial evaluation discussion. Options and eligible entitlements would be reviewed to promote consumer education and informed choice, including location, cost, insurance coverage, services, and faith based /cultural preferences.

Assistance filing for Social Security Disability (SSDI), and Supplemental Security Income (SSI), for when a community needs pooled trust is being implemented as part of the Medicaid process.
